Physical Requirements for the

2023-24 School Year

In May 2020, UIL passed a resolution concerning pre-participation physical exams in response to the COVID-19 pandemic. In compliance with UIL recommendations, LCISD will implement the following guidelines:

  • Parents of all athletes (7th-12th grade) and band members (9th-12th grade) will be required to complete the UIL Medical History form.

  • If the parent/guardian answers “Yes” to any of Questions 1-6, the student must get a new pre-participation physical for the 2020-21 and provide proof of the exam.

  • If the parent/guardian answers "No" to any of the Questions 1-6, and LCISD has a previous physical on file for the student, the student will not be required to get a new pre-participation physical exam.

  • All 6th grade students in Pre-Athletics will be required to get a pre-participation physical examination.

  • Any athlete/band member who does not currently have proof of a pre-participation physical exam on file with LCISD will be required to get a new pre-participation physical exam and provide proof of the exam.

  • All other UIL and LCISD forms will still be required.
